Abstract

The previous case report of methadone toxicity and alteration in cardiac rhythm has inhibited many healthcare providers from using this drug in children. Methadone has many advantages, such as being a highly lipophilic drug, larger volume distribution, highly variable elimination half-life ranging from 15-55 hours, and smaller volume use for larger opiate equivalents, which makes it a better option drug for opioid rotation. Children possess a variable clinical response to methadone, hence the need for rigorous monitoring of the side effects. We illustrated 2 paediatric cases to highlight the needs of methadone in different circumstances, with successful conversion from conventional opiates.
